If you are a new patient, or have not been seen by our office since July 1, 2014, please register in our Patient Portal via the link sent to your email address provided when your appointment was made. Once you are registered, click on each of the tabs at the top of the page and fill out the information such as medical history and medications. The only tab you do not need to fill out is the insurance tab.
If you did not provide our office with an email address or are otherwise unable to fill your information out online, please print out the relevant forms and bring a completed copy to your appointment.
