Overview

Crenshaw & Belani, MD, PC ("we", "us", "our") respects your privacy. This policy explains how this website collects, uses, and protects information. Protected Health Information (PHI) handled in the course of clinical care is governed by our HIPAA Notice of Privacy Practices, which is provided at your first visit.

Information we collect on this website

  • Contact details you provide through forms (name, phone, email, preferred appointment information).
  • Standard server logs (IP address, browser, pages visited) for security and operations.
  • Two values stored in your browser’s local storage: cea-cookie-consent, which records that you have seen our cookie notice, and cea-do-not-sell, which records a Do Not Sell opt-out if you choose to make one. This website does not use analytics, advertising, or session-recording technologies.

How we use information

  • To respond to appointment requests and questions you send us.
  • To operate, secure, and improve the website.
  • To comply with applicable laws and our professional obligations.

Global Privacy Control

We honor the Global Privacy Control signal. If your browser sends a GPC signal, we treat that as a valid opt-out from the sale or sharing of personal information and do not set non-essential cookies.

Do Not Sell or Share My Personal Information

We do not sell personal information for money. To opt out of any sharing for cross-context behavioral advertising, visit our Do Not Sell My Information page.

Third parties

This site loads content from third-party providers. Google Fonts delivers the typography used across the site. Google Maps is embedded on our Home, Contact, and Locations pages to display our office location. YouTube videos are embedded on our Cataract Surgery page for patient education; these use YouTube’s privacy-enhanced (youtube-nocookie.com) embed format. Loading any of these may connect your browser to Google or YouTube and transmit technical information such as your IP address, browser type, and the page you are viewing. That information is handled under Google’s privacy policy, and these providers may set their own cookies or use browser storage on the pages where their content appears.

Your rights

You may request access to, correction of, or deletion of personal information we hold about you by contacting our office. Residents of jurisdictions with comprehensive privacy laws (including Virginia) have additional rights described in those laws.

Children

This website is not directed to children under 13 and we do not knowingly collect information from them online.
